Our Medication Management Outpatient Services focus on providing expert care and oversight for individuals requiring pharmacological interventions for behavioral health conditions. We offer specialized consultations with experienced licensed providers who assess clients' unique needs and develop personalized medication plans. Medication management is integral to our holistic approach to mental health care. Providers collaborate closely with other clinical staff within our organization, ensuring a coordinated treatment plan incorporating therapy and other non-pharmacological interventions when appropriate. By working together as a team, we aim to optimize treatment outcomes and support our client's overall mental wellness.
We welcome referrals from individuals, families, medical providers, Clatsop County Court (DUII, Community Court, Mental Health Court), DHS (children and families), the Department of Justice (probation, post-prison supervision), and others. Our providers are specialists in working with acute and complex mental health and substance use disorders. Our provider team has limited capacity. Not all referrals will be assigned to a provider. Our team works to ensure bi-directional referral follow-up and communication. Please call the Enrollment Center at (503) 325-5722, Monday through Friday 9:00 a.m. to 5:00 p.m., to access our services or follow up on a referral. For new referrals - Download our referral form below and fax the completed form to (503) 325-8483.
We are happy to serve our community partners by offering one-time consults to other prescribing medical professionals when needed. Please download our Psychiatric Consultation Referral Form and return it via fax to (503) 325-8483.
